Subject: Revised Standard Flood Hazard Determination Form

The purpose of this Informational Memorandum is to communicate recent changes to the Federal Emergency Management Agency (FEMA) Standard Flood Hazard Determination Form (SFHDF). The SFHDF has been changed from FEMA Form 81-93 to FEMA Form 086-0-32 and the form has been revised. FEMA will allow users a six-month adjustment or until October 31, 2012 for final adoption of the form. The new form has an expiration date of May 30, 2015. All institutions should adopt appropriate procedures to ensure compliance. FEMA Form 086-0-32 can be accessed from the FEMA Web Site at:
